O fornecimento do cart\u00e3o ser\u00e1 concedido mediante apresenta\u00e7\u00e3o do formul\u00e1rio de solicita\u00e7\u00e3o apropriado, devidamente preenchido e assinado pelo servidor solicitante, discriminado o n\u00famero de deslocamentos di\u00e1rios necess\u00e1rios ao cumprimento do trajeto RESID\u00caNCIA &#8211; TRABALHO &#8211; RESID\u00caNCIA e seus respectivos valores praticados pela companhia de transporte respons\u00e1vel, acompanhado de c\u00f3pia do comprovante de endere\u00e7o. Cada servidor receber\u00e1 um PASS CARD que ser\u00e1 alimentado mensalmente com cr\u00e9dito equivalente \u00e0 sua despesa com o deslocamento supramencionado naquele per\u00edodo. O benef\u00edcio n\u00e3o usufru\u00eddo durante o per\u00edodo de vig\u00eancia deste Acordo Coletivo ser\u00e1 cancelado, n\u00e3o podendo este direito ser acumulado para o ano seguinte e\/ou, em hip\u00f3tese alguma, ser transformado em remunera\u00e7\u00e3o ao servidor.<\/span><\/p>\n<p><\/label><\/p>\n<p class=\"MsoNormal\" style=\"margin: 0cm 0cm 0.0001pt; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\"><br \/><strong><br \/><label class=\"tituloClausula\">CL\u00c1USULA D\u00c9CIMA QUARTA &#8211; INCENTIVO AO SERVIDOR ESTUDANTE:\u00a0<br \/><\/label><\/strong><\/p>\n<p><\/span><\/p>\n<p><label class=\"descricaoClausula\"><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\"> a) Ao servidor estudante ser\u00e1 assegurado o abono de sua aus\u00eancia ao trabalho durante o hor\u00e1rio de presta\u00e7\u00e3o de exames curriculares, desde que avisado \u00e0 Diretoria do CRP-11 com no m\u00ednimo 48 (quarenta e oito) horas de anteced\u00eancia e subordinado \u00e0 comprova\u00e7\u00e3o posterior pelo servidor no mesmo prazo, em ambos os casos, por escrito. N\u00e3o havendo a comprova\u00e7\u00e3o o per\u00edodo de aus\u00eancia ser\u00e1 computado como falta do servidor.\u00a0<strong>b) F\u00c9RIAS DO SERVIDOR ESTUDANTE:<\/strong> O CRP-11 conceder\u00e1 f\u00e9rias de seus servidores estudantes e aos servidores com filhos\/dependentes menores de 18(dezoito) anos, estudantes regularmente matriculados em institui\u00e7\u00e3o de ensino oficial, preferencialmente, em per\u00edodo que coincida com per\u00edodo de f\u00e9rias escolares. O referido benef\u00edcio dever\u00e1 ser solicitado pelos servidores \u00e0 Diretoria do CRP-11, no prazo m\u00ednimo de 60 (sessenta) dias de anteced\u00eancia, por escrito e a solicita\u00e7\u00e3o dever\u00e1 estar acompanhada da comprova\u00e7\u00e3o de efetiva matr\u00edcula naquele per\u00edodo.<\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\"> <\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\">Par\u00e1grafo \u00fanico. Para fins do que prev\u00ea o caput, ser\u00e3o beneficiados por per\u00edodo de f\u00e9rias, assim entendido, os meses de janeiro, julho e dezembro. A feitura da escala de f\u00e9rias poder\u00e1 ocorrer com acordo m\u00fatuo entre o conjunto de servidores para estabelecer revezamento plaus\u00edvel no gozo das f\u00e9rias de que trata este caput, por meio de apresenta\u00e7\u00e3o de propostas para a Diretoria para delibera\u00e7\u00e3o. Havendo acordo favor\u00e1vel ao conjunto dos servidores e que n\u00e3o traga preju\u00edzo para a administra\u00e7\u00e3o em seu funcionamento, ser\u00e1 encaminhada desta forma a escala de f\u00e9rias. Fica autorizado o desconto de at\u00e9 10% (dez por cento) do valor da mensalidade cobrada pelo plano privado de sa\u00fade na folha de pagamento de cada servidor que aderir espontaneamente ao conv\u00eanio mediante preenchimento e assinatura de formul\u00e1rio e de at\u00e9 40% (quarenta por cento) da mensalidade cobrada por cada dependente oficial, para os servidores que os inclu\u00edrem no seu contrato.<\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\"> <\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\">\u00a7 1\u00ba. O CRP-11 manter\u00e1 os dependentes n\u00e3o oficiais no conv\u00eanio de assist\u00eancia de sa\u00fade ofertado pelo Conselho, por\u00e9m, os mesmos dever\u00e3o arcar com 100% (cem por cento) dos custos do respectivo plano.<\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\"> <\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\">\u00a7 2\u00ba. Se por algum motivo o servidor deixar de arcar com a sua parcela ou de seus dependentes no custeio do plano de assist\u00eancia m\u00e9dica, por qualquer motivo que seja, fica desde j\u00e1 autorizado o CRP 11 realizar o cancelamento do seu plano e de seus dependentes.<\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\"> <\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\">\u00a7 3\u00ba. O servidor \u00e9 obrigado a pagar a sua parte no custeio do plano de sa\u00fade e de seus dependentes, ainda que n\u00e3o esteja recebendo sal\u00e1rios diretamente pelo CRP\/11, como no caso de licen\u00e7a m\u00e9dica pelo INSS, quando ficar\u00e1 obrigado a recolher aos cofres do CRP\/11 a parte que lhe cabe, sob pena de ser cancelado o plano de sa\u00fade, na forma do autorizado pelo \u00a7 2\u00ba acima.<\/span><\/p>\n<p style=\"margin-right: 0cm; margin-left: 0cm; font-size: 12pt; font-family: &quot;Times New Roman&quot;, serif;\"><strong><span style=\"font-size: 10.5pt; font-family: Arial, sans-serif;\"> <\/span><\/strong><span style=\"font-size: 10.5pt;
